9/3/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Client arrived to session on time and appeared to be having a conversation with father as QMHP-C entered. Client started to shut down as QMHP-C implied his behaviors. Client was refuses to express the situation and continued to respond nothing. Client was shutting down as QMHP-C asked questions. Client continues to struggle with engaging appropriate and showing positive self-esteem. Client refuses to be support by others and continued to shut down when getting assisted. Client struggled with having confidence when ask to complete a task as evidenced by Client stating "I Canât" and walked away from the activities. 2. Behavior: Client struggled with having confidence when ask to complete a task as evidenced by Client stating "I Canât" and walked away from the activities. While stating he could not do this he was crying and screaming to the top of his lungs. 1.Intervention: QMHP-C introduced self as Client and discussed methods for building therapeutic relationship through open communication and rapport-building exercisesâ-C addressed Clientâs behavioral issues on self-images. QMHP-C recommended Client to express himself and refrain for being nonchalant. 2. Intervention: QMHP-C recognized Clientâs positive mood and behaviors and encouraged him to maintain his current mood with self-checks and positive self-talk. QMHP-C encouraged Client to utilize his ability of being honest and taking responsibility. QMHP-C discussed with Client on the ability to manage behaviors according to the classroom expectations with effective self-monitoring. 1.Response: Client was receptive of feedback given and verbalized an understanding of advice. Client acknowledged feedback and was able to express insight and spoke clearly. Client was able to successfully follow directions and implemented skills without difficulties. 2. Response: Client verbalized to QMHP-C on how he continues to try not to get in trouble on a daily basis by his mother but at times his sister is always âpissingâ him off which makes him get in trouble.
